多臓器転移をきたしたS状結腸原発絨毛癌の1例

症例は38歳の男性で，便通異常と腹痛で近医を受診した．下部消化管内視鏡検査でS状結腸に全周性腫瘍を認め，生検にてGroup5，adenocarcinoma （por）であったためS状結腸癌の診断で当院受診した．CTで肝・右副腎・大動脈周囲リンパ節など複数臓器の腫瘍性病変を認めた．大腸癌・多臓器転移と診断したが消化管狭窄症状があったため結腸切除を行った．手術の際，腹膜播種病変も認めた．病理検査の結果，結腸原発絨毛癌と診断した．術後，大腸腺癌に基づいた化学療法（mFOLFOX6+Bevacizumab）を施行したが癌は進行し，急激に肝機能が悪化し術後59日目に肝不全で永眠した．消化管原発絨毛癌はま...
