広範囲解離性大動脈瘤に対してエントリー閉鎖後急性期に生じた切迫破裂
症例は41歳，Marfan症候群の男性．36歳時に急性大動脈解離（DeBakey I型）に対して大動脈基部置換術と弓部大動脈人工血管置換術を施行された．5年後に，下行大動脈に残存する解離性大動脈瘤が拡大傾向となり治療目的に当科に紹介された．下行大動脈のエントリー閉鎖目的にステントグラフト治療を行ったが，術後15日目に突然の背部痛が出現し，CT検査にて偽腔拡大所見を認め，切迫破裂と診断した．緊急で胸腹部大動脈人工血管置換術を施行し，術後は対麻痺なく経過良好であった．その後のCT検査にて肋間動脈からのtype IIエンドリークによる瘤径拡大を認め，原因と思われた肋間動脈のコイル塞栓とn-butyl...

| Abstract | 症例は41歳，Marfan症候群の男性．36歳時に急性大動脈解離（DeBakey I型）に対して大動脈基部置換術と弓部大動脈人工血管置換術を施行された．5年後に，下行大動脈に残存する解離性大動脈瘤が拡大傾向となり治療目的に当科に紹介された．下行大動脈のエントリー閉鎖目的にステントグラフト治療を行ったが，術後15日目に突然の背部痛が出現し，CT検査にて偽腔拡大所見を認め，切迫破裂と診断した．緊急で胸腹部大動脈人工血管置換術を施行し，術後は対麻痺なく経過良好であった．その後のCT検査にて肋間動脈からのtype IIエンドリークによる瘤径拡大を認め，原因と思われた肋間動脈のコイル塞栓とn-butyl-2-cyanoacrylate（NBCA）の瘤内注入療法を行い，最終的には瘤内の完全な血栓化が得られた．A型解離の術後やB型解離慢性期の残存解離の瘤化に対する治療方針については議論のあるところであり，症例ごとに最適な治療方針の検討が必要である．エントリー閉鎖後は本症例のように残存する開存偽腔からの吹き上げ血流により，瘤径が急速に拡大する可能性があり，より瘤径の大きい症例やステントグラフト留置部位より末梢にエントリーが残存する症例では，エントリー閉鎖後の厳重な経過観察と状況によって早期追加治療が必要であると考えられた． |
