子宮頸癌の巨大右室内転移に対して右室切開にて腫瘍摘除を行った1例
子宮頸癌からの心臓転移の症例は稀である．左鎖骨上リンパ節転移のため放射線治療を受けていた子宮頸癌の54歳女性の症例を報告する．患者は息切れを主訴に来院した．経胸壁心エコーで右室内に大きな腫瘤を認め，巨大腫瘤陥頓による突然死を防ぐため，準緊急手術を行った．冠動脈の後下行枝と左前下行枝に平行に右室を切開し，心内腫瘤を切除した．切除により，右室流出路閉塞や周術期の肺塞栓を予防し，死に至る可能性を回避した．心内腫瘤は子宮頸部扁平上皮癌であると診断し，退院後化学療法を施行した．術後3カ月目に行った心エコー検査で心臓転移の再発を認め，5カ月後に患者は死亡した．右室への心臓転移は，肺塞栓として現れることがあ...
