潰瘍様突出像を伴う慢性解離性大動脈瘤に対するステントグラフト内挿術時にIsolated left vertebral arteryを再建した1例

Abstract 胸部ステントグラフト内挿術時に,左椎骨動脈単独起始(Isolated left vertebral artery: ILVA)の閉鎖を余儀なくされる場合,ILVA再建の要否については統一した見解がない.症例は57歳女性.弓部大動脈にILVAのanomalyがあった.潰瘍様突出像を伴う血栓閉塞型Stanford B型急性大動脈解離に対し保存的降圧療法を行った.一年後に潰瘍様突出像の拡大と偽腔の拡大を認め胸部ステントグラフト内挿術の適応とした.ILVAの閉鎖が想定されたため,ILVA-左総頸動脈バイパス術,左総頸動脈-左腋窩動脈バイパス術を施行後に胸部ステントグラフト内挿術を施行した.術後経過は良好で合併症は認めなかった.ILVA再建は,脳梗塞,脊髄虚血の予防効果が期待され,解剖学的特性から手技が容易なため,積極的に再建した1例を報告する.
