原発性副甲状腺機能亢進症術後のQOL解析

Abstract 原発性副甲状腺機能亢進症は，様々な神経精神症状を伴う場合がある。一見自覚症状がない，いわゆる生化学型の患者ではこれらの症状があっても漠然とした非特異的症状であるので，加齢，日常生活のストレス，あるいは他の疾患の影響かと判断される可能性がある。これらの漠然とした症状が手術によって変化するかどうかを評価するために健康関連QOLの包括的尺度のSF-36が利用されてきたが，疾患特異的尺度としてPAS scoreが提唱されている。PAS scoreは簡便で迅速に計算できるので臨床の現場で利用しやすい。筋肉や平衡機能，睡眠などの症状の変化に対しても手術療法のQOL評価がされている。これらの評価によって，一見症状がない軽症の副甲状腺機能亢進症の場合でも手術によって症状が改善することが分かってきた。このことは，いわゆる無症候性副甲状腺機能亢進症の手術適応を考えるうえで大きな根拠になる。
