甲状腺全摘後の血清マーカー値の経時的変動分析に基づく甲状腺乳頭癌の予後の動的予測

Abstract 現在，甲状腺乳頭癌に対しては，腫瘍径，Ex，N，Mの評価に基づいて手術術式が選択され，術後は病理学的所見や術後の血清サイログロブリン（Tg）値に基づき，術後療法が選択されている。本論文では，Tg抗体（TgAb）陰性の乳頭癌においては甲状腺全摘後のTSH抑制下のTg値の変動から求めたTgダブリングタイム（TgDT）が多変量解析において最強の予後因子であることを紹介する。Tg値の変動が重要である。しかし，TgAb陽性患者においてはTg値の信頼性は低い。そこで，TgAb陽性の乳頭癌225症例の甲状腺全摘後のTgAb値の変動と予後との関係を調べると，術後２年以内に50％以上低下した症例は非低下症例より有意に良好であり，多変量解析において優れた予後因子であることが判明した。今後は，旧来のTNM，病理学的分化度による予後予測の時代から，血清Tg，TgAb値の経時的変動分析に基づく動的予後予測の時代へとパラダイムシフトが起こるものと予想される。
