重粒子線照射後胆管狭窄に対し,胆管ステント留置後に仮性動脈瘤破裂をきたした肝細胞癌の1例
症例は75歳男性.切除不能肝細胞癌に対して重粒子線治療(60 Gy/4 fractions)を施行した.照射4カ月後に胆管狭窄をきたし,肝左葉の胆管拡張を認めた.内視鏡的に胆管プラスチックステントを留置し,減黄が得られた.ステント留置2カ月後に,急性胆管炎をきたし入院となった.第1病日に吐血を認め,CT検査で中肝動脈の仮性動脈瘤破裂による胆道出血の診断となった.血管造影では中肝動脈に仮性動脈瘤の描出を認めた.シアノアクリレートで塞栓し止血が得られ,第35病日で自宅退院となった.重粒子線治療を含めた放射線治療や胆管ステントは仮性動脈瘤を形成することが報告されている.自験例は,肝細胞癌に対する重粒...
