Summary:Osteoma is a lesion that comprises growth of mature bone tissue. Peripheral osteoma can occur in the mandible at the inner or outer edge of the mandibular angle, the lower edge of the chin, or the lingual aspect of the molar region. We report a case of osteoma that arose on the medial and lateral aspects of the right mandibular angle. A 68-year-old man was referred to our hospital in 2017 due to restricted mouth opening and also swelling of the right mandible (the swelling was first observed in 1980). At the first visit, his facial appearance was asymmetrical, and a fixed mass with bone-like hardness measuring 47 mm × 34 mm was palpated at the right mandibular angle. Panoramic radiography and computed tomography revealed a pedunculated, bone-like radiopacity at the right mandibular angle. Bone scintigraphy showed abnormal nodular accumulation at the same location. A tumor resection was performed using a trans parotid approach under general anesthesia. The histological diagnosis was compact osteoma. Mouth opening increased to 42 mm from three months post-operation. The disease has not recurred as at two years post-operation.
