未成年喫煙者への禁煙支援に影響を与えるニコチン置換療法等の要因の検討

| Abstract | 目的 鎌倉保健福祉事務所において無償で未成年禁煙希望者にニコチンパッチ（以下パッチ）処方を行い，未成年喫煙者に対するニコチン置換療法（Nicotine Replacement Therapy 以下 NRT）と属性別の禁煙指導の効果について検討した。 方法 平成16年 4 月 1 日から平成17年 3 月31日の期間に禁煙相談を行った20歳未満の喫煙者を対象とした。分析項目は性別，年齢，喫煙開始年齢，過去の禁煙状況，喫煙本数，同居家族・友人の喫煙状況で，分析項目を聴取後ニコチンパッチ（商品名：ニコチネル TTS30）7 枚を無償で処方し，1 か月後，6 か月後に電話またははがきで喫煙状況，ニコチンパッチの使用有無を確認した。対象者数39人のうちニコチンパッチ使用状況の判明した者について SPSS11.0J を用い Fisher 正確検定と Mann-Whitney U 検定を行った。 結果 対象者数は39人で平均年齢16.4歳（14-18歳），平均喫煙期間は2.3年，平均喫煙開始年齢は13.3歳，平均喫煙本数は 1 日あたり12.8本であった。対象者39人のうち 1 か月後25人（64.1%），6 か月後21人（53.8%）について追跡可能で検討対象とした。39人中14人（intention to treat analysis; 35.9%）が 1 か月後に禁煙を継続していた。禁煙継続者14人と禁煙不成功者11人を比較したところ，ニコチンパッチ使用の有無において有意差を認めたが（P<0.05），他の項目で有意差を認めたものはなかった。6か月後は39人中10人（intention to treat analysis; 25.6%）が禁煙を継続しており，喫煙同居者の有無が禁煙継続において有意差を認めた（P<0.05）。ニコチンパッチ使用の有無は有意差を示さなかったが，サンプル数を増やす必要性が示唆された。またニコチンパッチ使用による副作用と思われる訴えは確認されなかった。 結論 今回の調査で未成年の禁煙支援に際して指導開始後 1 か月における有効性が示唆された。6 か月後では喫煙同居者が禁煙継続の阻害因子であることが推測された。未成年者への NRT を実施する研究において，対象者数の増加が今後の課題であるとともに，効率的に参加者を募集するためにはインフォームドコンセントや追跡方法に検討の余地があると思われた。 |
