今回, 我々は, 選択的近位迷走神経切離術後13年目に発見された胃癌の1例を経験したので報告する. 症例は74歳の男性で, 主訴は空腹時心窩部痛であった. 胃X線、検査, 胃内視鏡検査にて胃体部前壁にBorrmann III型様病変を認め, 生検にて腺癌と診断された. 腹部超音波, CT検査では他の部位には異常所見なく, 胃全摘術(D2), Roux-Y法再建術を施行した. 術後診断は, T2(SS), N0, P0, H0, Stage Ib, 根治度Aであった. 切除標本では胃体部前壁に4.0×2.5cm大のBorrmann III型胃癌を認めた. 組織学的にはpoorly differentiated adenocarcinoma, se, n(-), ly2, v1であった. 本邦において消化性潰瘍の外科的治療法の1つとして迷走神経切除術が普及し約20年になった現在, 術後の胃初発癌の発生頻度, 発生機序の解明が重要になってきたと思われる.
