オトガイ下皮弁による中咽頭再建の2例

オトガイ下皮弁(Submental Island Flap:SIF)による2例の中咽頭癌切除後再建について報告する。 症例1は74歳女性。前壁癌(cT2N1M0)切除後の再建に5×2.5cmのSIFを使用した。術後18日に常食摂取で退院した。 症例2は80歳男性。側壁癌(cT2N0M0)切除後の再建に5×3cmのSIFを使用した。術後に誤嚥性肺炎を2度起こしたものの,術後81日に常食摂取で退院した。 SIFの腫瘍学的安全性は口腔癌では明確だが中咽頭癌では検討されていない。しかし,中咽頭癌のLevel Ⅰへの転移率は口腔癌に比べてはるかに少ないため,その安全性は担保されると考えられた。...
