外科的治療により長期生存が得られた大網原発転移性消化管外間質腫瘍の1例

症例は63歳の男性で，心窩部痛を主訴に当院を受診．腹部エコーで肝右葉に巨大腫瘍を指摘され緊急入院した．腹部CT，血管造影検査で，同部に血流豊富な充実性腫瘍を認めた．15年前に大網平滑筋肉腫の既往があり，同腫瘍の肝転移と診断し，肝右三区域切除を施行した．病理組織学的検査は紡錘細胞が束状に配列し，免疫組織染色検査ではKIT陽性で，消化管間質腫瘍（Gastrointestinal stromal tumor）の肝転移と診断された．大網腫瘍も同様の結果であり，かつて大網の平滑筋肉腫と診断された腫瘍は，消化管外間質腫瘍（Extragastrointestinal stromal tumor）とあらためて...

Abstract 症例は63歳の男性で，心窩部痛を主訴に当院を受診．腹部エコーで肝右葉に巨大腫瘍を指摘され緊急入院した．腹部CT，血管造影検査で，同部に血流豊富な充実性腫瘍を認めた．15年前に大網平滑筋肉腫の既往があり，同腫瘍の肝転移と診断し，肝右三区域切除を施行した．病理組織学的検査は紡錘細胞が束状に配列し，免疫組織染色検査ではKIT陽性で，消化管間質腫瘍（Gastrointestinal stromal tumor）の肝転移と診断された．大網腫瘍も同様の結果であり，かつて大網の平滑筋肉腫と診断された腫瘍は，消化管外間質腫瘍（Extragastrointestinal stromal tumor）とあらためて診断された．肝切除6年半後に頭蓋底再発を認め腫瘍摘出術を施行し，現在スニチニブ内服中である．EGISTの予後や転移巣に対する治療方針はいまだ明らかでない．今後EGIST転移例を集積しGIST転移例との生物学的悪性度の違いを検討して，治療方針を明確にしていく必要がある.
