当院における再発・転移頭頸部扁平上皮癌に対するニボルマブの予後延長効果の検討

| Abstract | CheckMate 141試験の結果を受け当院では2017年10月より再発・転移頭頸部扁平上皮癌に対するニボルマブ治療を開始した．本研究では実臨床の場でニボルマブの予後延長効果が実現しているか検証した．対象は2010年1月から2019年10月に当院で再発・転移頭頸部扁平上皮癌に対して緩和的治療を施行された患者とし，ニボルマブ投与群，ニボルマブ保険収載前後群（2017年10月以降のニボルマブ投与機会の可否で2群に分けた）を抽出した．ニボルマブ投与群の1年生存率は58.9%，2年生存率は46.6%でありCheckMate 141試験と比較して良好な治療成績だった．ニボルマブ保険収載前後群では1年生存率は保険収載前群で43.9%，保険収載後群で70.5%，2年生存率はそれぞれ21.7%，57.2%と有意に保険収載後群で全生存率が延長していた（p < 0.001）．また，緩和的治療開始後1年以内のBSC（Best Supportive Care）移行率は保険収載前群で52.3%，保険収載後群で9.3%と保険収載後群で早期BSC移行率が低かった．以上より，当院でもニボルマブの予後延長効果が追証可能でありニボルマブ登場による緩和的薬物療法の治療選択肢の増加により早期にBSCへ移行する患者が減少した． |
