Abstract 【背景】有症状肺動脈血栓塞栓症(pulmonary thromboembolism; PTE)は医療行為中に発症する重篤な合併症である.これまでわれわれが経験した有症状PTE症例を振り返り今後のさらなる予防対策を検討することを目的とした.【対象と方法】2005年以降に経験した静脈血栓塞栓症患者600例中有症状PTE患者75例を検討対象とした.PTE発症場所は院内と院外に分けた.手術や外傷後の症例(外科系)とそれ以外(非外科系)に分け検討した.【結果】院内38例(外科系23例,非外科系15例),院外37例(医療を受けていた患者22例,医療を受けていなかった15例).PTE重症度cardiac arrest 2例(死亡2例),massive 13例(死亡1例),sub-massive 18例,non-massive 42例.院内外科系症例の発症時予防状況は,抗凝固施行中3/23(13.6%),間欠的空気圧迫施行中16/23(72.9%),弾性ストッキング着用中20/23(90.9%)であった.院内非外科系症例の発症時予防状況は,抗凝固施行中,間欠的空気圧迫施行中2/15(13.3%),弾性ストッキング着用中2/15(13.3%)であった.【考察】術後PTE発症の多くは抗凝固療法未施行であり,時間が経過してからでも抗凝固予防を考慮すれば有症状PTEを減少させられる可能性がある.ただし,院内発症PTEをすべて予防できるわけではなく,患者と医療者でのリスクの共有が必要である.
