ステント留置後の複雑分岐部病変に対してDual Wire PTCA Dilatation Catheter(SafecutTM) が有効であった1例
症例は72歳, 男性. 2006年に労作時狭心症に対して第2対角枝にシロリムス溶出性ステント(CYPHER)が留置されている. 1年後の確認造影にて左前下行枝中間部に新規病変を認め, また第2対角枝のステント近位側に再狭窄を認めた. 両病変に対してパクリタキセル溶出性ステント(TAXUS)が留置され, 第2対角枝分岐部にステントが留置された際に, ステントがone strut左前下行枝に突出しているのが血管内超音波にて確認された. 半年後に労作時狭心症を認めたことから, 冠動脈造影をしたところ, 左前下行枝の第2対角枝分岐部付近に90％の再狭窄を認めた. 複雑分岐部病変であり, 左前下行枝にス...

| Abstract | 症例は72歳, 男性. 2006年に労作時狭心症に対して第2対角枝にシロリムス溶出性ステント(CYPHER)が留置されている. 1年後の確認造影にて左前下行枝中間部に新規病変を認め, また第2対角枝のステント近位側に再狭窄を認めた. 両病変に対してパクリタキセル溶出性ステント(TAXUS)が留置され, 第2対角枝分岐部にステントが留置された際に, ステントがone strut左前下行枝に突出しているのが血管内超音波にて確認された. 半年後に労作時狭心症を認めたことから, 冠動脈造影をしたところ, 左前下行枝の第2対角枝分岐部付近に90％の再狭窄を認めた. 複雑分岐部病変であり, 左前下行枝にステント留置した場合には, 第2対角枝のステントのクラッシュが必要で, 第2対角枝の血流に影響を及ぼす可能性があったことから, ステント留置が困難と判断した. 今回, われわれは, 同病変に対してSafecutTMを用いてKBT(kissing balloon technique)を用いた経皮的古典的バルーン形成術(plain old balloon angioplasty; POBA)を行った. 術直後の冠動脈造影では標的病変部に限局性の冠動脈解離を認めたが, 慢性期の確認造影では修復されており, 再狭窄も認められず, 複雑分岐部病変に対してSafecutTMが有効であると考えられた. |
