Abstract 腹腔鏡による腎盂・尿管癌手術は，1991年の登場以来多くの施設で行われるようになった．かつては，低侵襲手術として腹腔鏡下手術が優先される傾向にあったが，近年再び，開創手術が見直されている．腎癌に対する腎摘除術において，腹腔鏡手術か開創手術か，ではなく，全摘除術か部分切除術かと議論されることに比較すると，きわめて対照的である．これは，腎摘除術が腹腔鏡手術で行われるのは，cT3a程度の腎癌までであり，比較的早期癌を対象とした手術であること，また，局所進行性腎癌になると合併切除の可能性などにより腹腔鏡下手術では切除困難と判断されるケースがほとんどであることによる．腎盂・尿管癌においては，局所進行癌（cT3，腎盂癌であればcT4でも）であっても，長期のoncological outcomeは別にして，surgical margin陰性を腹腔鏡で達成しようとすれば手技的には可能である．近年，リンパ節郭清の重要性が明らかになりつつあり，腹腔鏡による腎尿管全摘除術の長期成績が再評価されている．しかし，腹腔鏡下手術による低侵襲性の恩恵を受ける患者が多く存在することも事実であり，すべての症例で開創手術がよいわけでもない．適切な症例を選択し，適切な手術方法で行うことが腹腔鏡下手術においては必要である．本稿では，そのヒントとして腹腔鏡下腎尿管全摘除術の成績と課題（とくに長期予後，非再発率，膀胱再発）について検討した．
