脳機能温存に留意したグリオーマ治療
グリオーマは生活の質を保ちながら生存期間の延長を目指す疾患となった. 生活の質を保つには, 運動・感覚・言語機能のみならず, その人らしさを司る高次脳機能の温存が必要となる. 手術モダリティとしては覚醒下手術と電気生理学的モニタリングが脳機能の温存に有用である. 化学療法としてテモゾロミド, ベバシズマブはそれぞれ抗腫瘍効果と抗浮腫効果により脳機能維持のために有用である. 高齢者では放射線治療後に認知機能障害をきたしやすく, 短期治療や小分割照射が推奨される. また, NovoTTFは細胞増殖を阻害する治療法であり, その腫瘍抑制効果によって脳機能維持を期待できる治療法である....
