トランスアミナーゼに比しα-フェトプロテインの高値で発症し早期に肝硬変に移行したB型肝炎ウイルスキャリアーの1例
症例は21歳の女性.18歳の時定期検診でHBs抗原の陽性が判明したが,血清GOT, GPTが正常であったので経過観察中,21歳の時,吐気,嘔吐に続く食欲不振で発症し,GOT, 175KU, GPT 173KUの上昇に対し血清α-フェトプロテイン(AFP)は1,237ng/mlの高値を示した.その時点における生検肝組織で,壊死巣中ないし近接部位の残存肝細胞にAFP染色の陽性所見を得た.IFNβ 600～300×104単位28日間の投与でHBe抗原陽性からHBe抗体陽性へと転換し,GOT, GPT, AFPも低下したが完全に正常化しなかった.画像的には急性肝炎に合致したが,組織学的には発症の時点で...
