出血傾向を示した抗リン脂質抗体症候群の10歳女児例
症例は乳児期より出血傾向を認め, 10歳の時に血小板減少性紫斑病で入院し,精査の結果抗リン脂質抗体陽性SLEと診断しえた.患児の血小板数は6.0×103/μlと著減し, APTTの延長を示していた.ステロイドにて治療開始後血小板数は正常化した.梅毒血清反応の生物学的偽陽性反応,ループスアンチコアグラントおよび抗カルジオリピンIgG, IgM抗体は陽性であり抗リン脂質抗体症候群の診断基準を満たした.さらに抗核抗体陽性,蛋白尿,血尿を示すため,腎生検を施行した.腎組織像はループス腎炎(瀰漫性膜性糸球体腎炎)を示し, SLEと診断された.現在までステロイドによる治療をおこなっているが,検査上線溶系の...
