慢性関節リウマチの経過中に発症した好酸球性髄膜脳炎

症例は56歳男性. 11年前発症の慢性関節リウマチに対してPrednisolone 10mg/日の内服治療中であったが,約3日間の経過で徐々に意識レベルの低下をきたして入院した.髄液は黄色に混濁しており,好酸球が著増していた.末梢血の好酸球も遅れて増加し,不隠状態に進展した.寄生虫感染,中でも広東住血線虫感染も疑われたが,髄液中にも虫体を検出し得ず,免疫学的診断にても同定できなかった.
