非閉塞性肥大型心筋症の体表面電位図所見

Abstract | 肥大型心筋症患者における心電図変化の特徴を正常群との隔たりを表現するdeparture mapを作成して検討した.対象は心エコー図にて中隔厚13mmをこえる肥大型心筋症患者24名で, この疾患群を後壁厚13mm以下のA群16名と13mmをこえるB群8名とに分類した.体表面電位図は87誘導点より記録, 各誘導点のQRS波形につき, (1) QRS開始より10, 20, 30, 40, 50, 60mseoの各電位, (2) QRS波形の時間積分値AQRS, (3) 心室興奮到達時間VATを求めた.Departure index (DI) を〔 (各計測値一正常群より求めた平均値) /正常群より求めた標準偏差〕の式にて算出, DIの体表面分布をdeparture mapとして表現した.DI≧2を+2SDareaとし異常正領域と考えた.A群, B群の相違は50・60mseo, AQRS, VATにて明瞭であり, それぞれのdeparture map上の+2SD areaは, A群では左前胸部上方・側胸部上方に, B群では左前胸部から側胸部・背部にかけて広範に出現した.心筋の肥大部位がdeparture mapに表現され, その推定が可能と考えられた.また, 50, 60mseo, AQRS, VATの各departure map上の+2SD areaの面積と, 壁厚和 (中隔厚+後壁厚) は, それぞれr=0.63~0.81の相関を示し心筋肥大の程度も推定可能と考えられた.肥大型心筋症の心電図変化の特徴は, 心筋の肥大部位に対応した心起電力の増大と心室興奮伝播遅延と考えられた. |
