HEART's Original [症例] 負荷心電図にて著明なST低下を認めた正常冠動脈の1例

症例は67歳の女性.2005年1月,急性糸球体腎炎のため当院腎臓内科入院.安静時心電図で非特異的ST変化が認められたため虚血性心疾患の疑いで当科紹介となった.明らかな症状の発現はみられなかったが,精査のため心エコー, 運動負荷心電図, 運動負荷タリウム心筋シンチグラフィなどを行ったところ, 運動負荷心電図にてII, III, aVF,V3～4誘導においてdown slope型の著明なST低下が認められた.虚血性心疾患の可能性を考え,冠動脈造影検査を施行したが,左右の冠動脈に有意狭窄はみられなかった.その他,ホルター心電図や各種ホルモン検査,MIBGシンチグラフィも施行したが明らかな異常所見は認...

Abstract 症例は67歳の女性.2005年1月,急性糸球体腎炎のため当院腎臓内科入院.安静時心電図で非特異的ST変化が認められたため虚血性心疾患の疑いで当科紹介となった.明らかな症状の発現はみられなかったが,精査のため心エコー, 運動負荷心電図, 運動負荷タリウム心筋シンチグラフィなどを行ったところ, 運動負荷心電図にてII, III, aVF,V3～4誘導においてdown slope型の著明なST低下が認められた.虚血性心疾患の可能性を考え,冠動脈造影検査を施行したが,左右の冠動脈に有意狭窄はみられなかった.その他,ホルター心電図や各種ホルモン検査,MIBGシンチグラフィも施行したが明らかな異常所見は認められず,運動負荷試験偽陽性例と判断した.運動負荷心電図は特に女性において偽陽性例が少なからず存在するといわれている.その原因は定かではなく,本症例においてもその原因ははっきりしなかったが,本症例では貧血とそれに伴う負荷中の著明な洞性頻脈があり,心拍数依存性にST低下が著明に現れた可能性が考えられた.最近の大規模臨床試験では,女性において運動負荷心電図のST低下は長期予後に関連しないとする報告がなされており,本症例においても運動負荷試験のST低下は予後不良を示唆する所見ではないと思われた.
