一般健診におけるLDL-C(直接法,F式法)を用いた脂質代謝異常判定の問題点

Abstract | 目的:健診受診者を対象とし,LDL-C直接値とFriedewald(F)式による計算値を対比し問題点を検討した. 方法:当院健診センターで受診した3,545名(男:2,146名,女1,399名,平均年令50.9±6.7歳)を対象とし,統計学的解析でF式におけるTGの係数を推定した. 結果:LDL-C直接値はF式計算値より有意に高値で,両者間には強い相関があった.LDL-C直接値はTC,non HDL-Cと強い相関を認めた.直接値とF式値との%誤差[(F-LDL-C)-(D-LDL-C)/(D-LDL-C)×100]とTG値は有意に相関し,TGが高くなるに従い負の方向に低下した.TGとVLDL-Cとの比から,F式TG係数は,0.16±0.06であり,TC,HDL-C,TGの一次多項式からTG係数は0.1に近似した.またVLDL-CとTGとの散布図から求めた直線回帰から,TG係数は0.12であった.またTG値が低い場合に係数のばらつきが大きくなったが,TGが100~350mg/dLの範囲では直接値とF式値の一致度は高かった. 結論:LDL-C直接値とF式計算値は強い相関を認めたが,TGが高くなるに伴って,直接値はF式計算値より高くなった.TG係数はTGの値に関連することが示唆され,TGが低い場合,TG係数のバラつきが大きくなった.健診ではTG値に注意してF式値を用い,TC測定も必要である. |
