| Abstract | ［背景］近年，各国において医師の労働環境調査が実施され，労働環境と医師の精神衛生状態，そして，こうした環境で生み出された医療の質について検討が行われている．若手心臓血管外科医の労働環境について，詳細なアンケート調査と解析を行い，労働環境改善のためのKey factorを探った．［方法］本アンケート調査は，満40歳以下の若手心臓血管外科医を対象に，労働環境について筆記型アンケートおよびWeb入力型アンケートを行い，修練状況および労働状況の調査（手術業務，周術期業務，労働睡眠時間，資格の申請や更新，将来展望とMotivation，給与，休暇，PrivateのQuality of life，精神衛生）と，労働環境に対する満足度調査を行った．労働環境に関する基礎的16項目（年齢，卒後年数，性別，Subspecialty，外科専門医，心臓血管外科専門医，所属施設，所属施設勤務日数・当直数，所属病院外の勤務日数・当直数，年収，時間外労働時間，時間外申請時間，申請時間との差，ICU専従医によるICU管理の有無）をもとに，労働環境への不満に対する単変量解析，多変量解析を行った．［結果］対象会員1,304人から計327件の回答が得られ，回答率は25.1%であった．回答者の平均卒後年数は8.5±3.5年，男性292人（89.3%），女性34人（10.4%），非回答1人であった．満足度調査ですべての回答に不満がない者は全回答者の14.2%に留まった．一方，満足の回答は，手術業務の項目で最多であった（34.6%）．多変量解析において，労働環境のいずれかの不満項目に対して，年齢の増加，卒後年数の増加，女性，外科専門医，血管外科専門，大学病院勤務，所属施設勤務日数の増加，所属病院外の勤務日数の増加，時間外労働時間の増加は有意に不満に寄与する因子であり，年収の増加，心臓血管外科専門医，ICU専従医によるICU管理であることは有意に不満抑制的な因子であった．［結語］本アンケート調査結果の解析より，若手心臓血管外科医の労働環境において，不満に寄与する因子が特定された．医療の質の観点からは，医師への過剰負担を減らすことが望まれ，心臓血管外科領域の重症患者を安全に診療する枠組みとしては，心臓血管外科内および集中治療医や他職種とのInterdisciplinary teamの構築が期待される．労働環境の中で相関し合う心臓血管外科医療の享受者（患者）および提供者の実態把握には，満足度および自己研鑽としての対価，医療の質の定量評価を含めた多面的な解析が，今後望まれる． |
