実臨床における新しい尋常性乾癬外用療法であるフォーム剤の可能性（DEFENDER study）

| Abstract | 目的：実臨床における新しい配合外用薬［活性型ビタミンD3誘導体：カルシポトリオール（Cal）とコルチコステロイド：ベタメタゾンジプロピオン酸エステル（BDP）］フォーム剤の有効性を検討する．方法：日本全国の開業医を主な対象としたリアルワールドの診療情報データベースを用い，2021年6月1日〜2023年6月30日にCal/BDPフォーム剤の使用を初めて開始し，治療前後（0週時，4週時）の臨床評価（PASIスコア，IGAスコア，BSAスコア）が揃っている尋常性乾癬患者を調査対象として検討した．結果：実臨床の尋常性乾癬治療において，新規例そして切替例の前治療の外用薬に関係なく，更に軽症から重症まで疾患重症度にも関係なく，Cal/BDPフォーム剤は4週間の塗布でいずれの評価項目でも統計学的に有意な改善を示した．尋常性乾癬患者のQOLを低下させる「かゆみ」についても，Cal/BDPフォーム剤は新規例にも切替例にも統計学的に有意な改善がみられた．また，今回の実臨床におけるCal/BDPフォーム剤のデータでは，IGA×BSAスコアとPASIスコアの間に強い相関がみられ，日常診療において尋常性乾癬の疾患活動性を容易に評価できる有用な指標になることが示唆された．結論： 1日1回の塗布で済み，広範囲の病変部にも容易に塗布できるために患者のアドヒアランスの向上が期待でき，主薬の皮膚透過性も高いために速やかに高い効果が得られることが報告されているCal/BDPフォーム剤は，今回，日本の実臨床における尋常性乾癬の外用療法においても，重症度や新規・切替例に関係なく統計学的に有意な皮膚症状の改善を示した．日常診療においては，このようなエビデンスを患者と共有し，適切な治療選択をすることが重要であると考えた． |
