SLE様の病像で発症したCrohn病の一例
全身性エリテマトーデス(SLE)様の病像で発症したCrohn病(CD)症例を報告する.症例は37歳,男性.発熱,口腔内アフタ,咽頭痛,多発性関節痛のため来院.血液検査上,抗核抗体陽性,抗DNA抗体陽性, LE細胞陽性,白血球減少,リンパ球減少,さらに約1年間のてんかん発作のエピソードなどからSLEと診断された. prednisolone(PSL)の投与により,症状は改善, SLEとしての検査異常は正常化したが,白血球減少と口腔内アフタは持続し完治することはなかった.初診より10カ月後,吐血.内視鏡で食道潰瘍が確認された.その後はPSLにcyclophosphamide(経口50mg),あるいは...

| Abstract | 全身性エリテマトーデス(SLE)様の病像で発症したCrohn病(CD)症例を報告する.症例は37歳,男性.発熱,口腔内アフタ,咽頭痛,多発性関節痛のため来院.血液検査上,抗核抗体陽性,抗DNA抗体陽性, LE細胞陽性,白血球減少,リンパ球減少,さらに約1年間のてんかん発作のエピソードなどからSLEと診断された. prednisolone(PSL)の投与により,症状は改善, SLEとしての検査異常は正常化したが,白血球減少と口腔内アフタは持続し完治することはなかった.初診より10カ月後,吐血.内視鏡で食道潰瘍が確認された.その後はPSLにcyclophosphamide(経口50mg),あるいはメソトレキサート(7.5mg/日/週)の併用を行ったが口腔内アフタ,食道潰瘍は持続した.初診より5年7カ月の時点でPSLも中止した.初診より9年2カ月を経過して体重減少,下痢,下血などの下部消化管症状が出現,入院.下部消化管造影ならびに内視鏡所見よりCDと診断された. CDや潰瘍性大腸炎では多発性関節痛を始めとして膠原病様の症状が出現することが知られている.本例は, CDのような非膠原病性疾患であっても時にSLE分類予備基準を満足することがありうることを示している. |
