慢性関節リウマチおよび若年性関節リウマチ患者の顎関節の臨床症状とこう合状態に関する調査

The purpose of this study was to investigate the association between clinical signs of the temporomandibular joint (TMJ) and occlusion in chronic rheumatoid arthritis (RA) patients, and in juvenile rheumatoid arthritis (JRA) patients. The subjects were 72 RA patients (7 males and 65 females; mean age, 50.5 years) and 20 JRA patients (5 males and 15 females; mean age, 14.4 years). The results were as follows:1. In both the RA and JRA patient groups, the prevalence of TMJ dysfunction was higher in patients with malocclusion than in patients in the normal occlusion group. In addition, the prevalence of TMJ dysfunction was higher in patients with maxillary protrusion and open bite.2. Many RA patients had maxillary protrusion, while many JRA patients had open bite.3. No significant difference was found between the clinical signs of the TMJ and occlusion in both the RA and JRA patient groups. 本研究の目的は慢性関節リウマチ (RA) および若年性関節リウマチ (JRA) 患者の顎関節の臨床症状と咬合状態との関係を検討することである。RA患者72名 (男性7名, 女性65名, 平均年齢50.5歳) およびJRA患者20名 (男性5名, 女性15名, 平均年齢14.4歳) を研究対象とし, 以下の結果を得た。1. RAおよびJRA患者とも不正咬合群における顎関節の臨床症状の発現頻度は正常咬合群より高かった。また不正咬合群のなかでも上顎前突や開咬での発現頻度が高い傾向を示した。2. RA患者では上顎前突が, JRA患者では開咬が多かった。3. RA患者とJRA患者において, 顎関節の臨床症状と咬合との間に, 有意な差は認められなかった。
