脾原発多発性sclerosing angiomatoid nodular transformationの1例
症例は61歳の男性で，食思不振と体重減少を認め，CTで最大径12cmの多発脾腫瘤を指摘された．血液疾患を疑ったが，画像診断および骨髄生検で確定診断に至らず，組織診断および治療目的に開腹脾臓摘出術を施行した．各腫瘍径は7.5×5.5cm，4.0×4.0cm，3.5×4.0cm，3.0×10.0cmであった．免疫染色でcord capillary・sinusoid・small veinsの3種の血管成分の混在所見を認め，病理組織学的にsclerosing angiomatoid nodular transformation（SANT）と診断した．SANTは2004年に初めて報告された脾臓に発生する...

| Abstract | 症例は61歳の男性で，食思不振と体重減少を認め，CTで最大径12cmの多発脾腫瘤を指摘された．血液疾患を疑ったが，画像診断および骨髄生検で確定診断に至らず，組織診断および治療目的に開腹脾臓摘出術を施行した．各腫瘍径は7.5×5.5cm，4.0×4.0cm，3.5×4.0cm，3.0×10.0cmであった．免疫染色でcord capillary・sinusoid・small veinsの3種の血管成分の混在所見を認め，病理組織学的にsclerosing angiomatoid nodular transformation（SANT）と診断した．SANTは2004年に初めて報告された脾臓に発生する稀な良性病変であり，多くは単発かつ無症状で偶発的に発見される．本邦では多発かつ10cmを超えた報告例はない．今回われわれは，食思不振を契機に発見された多発性SANTの1切除例を経験したため，若干の文献的考察を加えて報告する． |
