尿路結石症の発生原因に対する内分泌学的検討 第IV報 カルシウム制限および負荷試験による高カルシウム尿症における病態生理の研究

前報のごとくCa摂取量がほぼ日常通りの条件下で, 尿路結石症のCa代謝について知り得ることには限界があつたため, Ca制限及びCa負荷によるCa摂取量に条件を加え検討をした. Abstract 前報のごとくCa摂取量がほぼ日常通りの条件下で, 尿路結石症のCa代謝について知り得ることには限界があつたため, Ca制限及びCa負荷によるCa摂取量に条件を加え検討をした. (1) 1日200mg以下のCa制限食を4日間したCa制限試験では, 尿中Ca排泄量 (U-Ca) は高Ca尿症群 (HC) においてのみ著明に低下, 尿中 cyclic AMP (U-cAMP) もHCにのみ有意な低下をみた. (2) 1g相当のCa負荷試験ではU-Ca, 血清Ca値は対照群 (CG), 正常Ca尿症群 (NC), HCの3群共に極めて大きい上昇をみたが, HCの上昇は他の2群よりも有意に大きかつた. この事からHCの原因は日頃のCa摂取量が多い他に, 腸管Ca吸収率が異常に多いことが推察された. Ca値の上昇と共にCGやNCではU-cAMPやPTHは低下したがHCでは低下せず, この理由はHCでは前報のごとく上皮小体機能が抑制されているため, たとえCa値が上昇しても余り低下しないものと考えられた. またその代償として, 血清Ca値を下げる目的で血清カルチトニン値が, HCにおいてのみ有意に上昇したものと推察される. (4) HCの中にはCa制限にてもU-Caが低下せず, U-cAMPも上昇しない症例がみられた. この症例は他のHCに比べ, U-cAMPやPTHが高く, 血清Caが低かつた. またCa負荷ではU-cAMPが著しく低下した. この事からこの症例では腎尿細管Ca再吸収量が高Ca尿症の原因で, いわゆる「腎Ca喪失型」と考えられ, その結果続発性上皮小体機能亢進症の状態を呈し, 一方他の大半のHC症例は「腸管吸収型」と考えられた.
