Abstract Tanawuttiwat et al present a case study of a 52-year-old woman, who was presented with a suspected myocardial infarction with mirror-image dextrocardia, a condition in which the anterior-posterior relationship of the various parts of the heart are normal, but their right/left orientation is reversed. Mirror-image dextrocardia is almost always associated with situs inversus of the abdominal organs. In dextrocardia patients with acute myocardial infarction, diagnosis and treatment via coronary angiography and percutaneous intervention are challenging because the mirror-image position of the coronary ostia and the aortic arch complicate orientation and cannulation. The patient underwent successful percutaneous coronary intervention with bare-metal stem placement in the mid-right coronary artery. Medical therapy was optimized, and she was discharged home in stable condition.
