妇产科医院临床药师开展药学服务的实践与体会

R97; 目的:探讨妇产科医院临床药师开展药学服务的模式和方法.方法:根据妇科、产科患者疾病特点,临床药师逐步开展药学查房、药学会诊、药学服务路径等工作.结果:针对典型案例,临床药师通过开展多样性的药学服务,为临床医护人员提供了药学支持,优化了患者的药物治疗方案,并协助医护人员处置了药品不良反应.结论:积极开展药学服务,有利于加强临床药师与临床医务工作者的协作,可促进药物的合理应用,保障患者的用药安全....

OBJECTIVE： To probe into the model and method of developing pharmaceutical care by clinical pharmacists in Obstetrics and Gynecology Hospital. METHODS： According to the disease characteristics of patients in obstetrical and gynecological department,the clinical pharmacists developed multiple services including pharmaceuticalward round,pharmaceutical consultation and pharmaceutical care pathways. RESULTS： In view of the typical case,through developing various pharmaceutical care,the clinical provided pharmaceutical support for the clinical medical and nursing staff,optimized patients＇ therapy regimen and assist the medical and nursing staff in dealing with adverse drug reactions.
