Bibliography:OBJECTIVE: To investigate the clinical practice of the CYP2C9 and VKORC1 genotype detection in warfarin individualized anticoagulation participated by clinical pharmacists. METHODS: Retrospective analysis was conducted on one successful case participared by clinical pharmacists based on pharmacogenomics of warfarinindividualized medication. RESULTS: According to the genetic detection results,the clinic pharmacists formulated and adjusted the warfarin anticoagulation dosage for the patient, and then provided the pharmaceutical care.CONCLUSIONS: Warfarin individualized medication based on pharmacogenomics by clinical pharmacists can provide scientific basis for warfarin adjustment,which can reduce the blindness of traditional experience and potential risks,increase patients' compliance and improve the success rate of anticoagulation treatment.
