促性腺激素释放激素类似物治疗中枢性性早熟女童及对骨代谢的影响

Abstract_FL Objective To observe the changes of bone metabolic markers in girls with central precocious puberty (CPP) treated by gonadotropin-releasing hormone analog(GnRHa) and to investigate its effect on bone metabolism. Methods 30 girls with first onset of idiopathic CPP(CPP group) in the pediatric endocrinology outpatient department of our hospital from June 2013 to December 2014 were selected. Serum levels of N-mid fragment of osteocalcin(N-MID),β-crosslaps and total procollagen type I N-terminal propeptide(PINP) before GnRHa treatment and after 6-month GnRHa treatment were measured with ELISA. The detec-tion results were compared with those in 30 age-matched girls undergoing physical examination(control group) at the same period. Results (1)The levels of serum N-MID,β-Crosslaps and PINP before GnRHa treatment in the CPP group were significantly higher than those in the control group,the differences were statistically significant[(109.3±27.4 )ng/mL vs.(73.7±11.3)ng/mL;(1.623 ±0.292)ng/mL vs. (1.191 ±0.246)ng/mL;(847.63 ±114.88)ng/mL vs. (642.71 ±152.99)ng/mL,P<0.05]. (2) The levels of serum N-MID,β-Crosslaps and PINP after 6-month GnRHa treatment in the CPP group were (77.4±11.7),(1.204±0.248),(617.89± 163.38)ng/mL respectively,which were significantly decreased compared with before treatment,the differences were statistically significant(P<0.05),but there were no statistical differences compared with those in the control group (P>0.05). Conclusion The GnRHa therapy can inhibit the excessive hyperfunction of osteoblasts and osteoclasts. But the evidence of short-term therapy influ-encing the bone quantity accumulation is lacked ,while the influence of long-term therapy on bone metabolism still needs to be further observed.
