L'athérosclérose est une pathologie artérielle correspondant à une pathologie inflammatoire chronique liée à l'interaction entre les lipoprotéines modifiées, les cellules inflammatoires, macrophages dérivés des monocytes circulants et lymphocytes T, et les éléments cellulaires de la paroi artérielle. Cette inflammation chronique conduit à un processus réactionnel cicatriciel de la paroi artérielle impliquant les cellules musculaires lisses et la production de matrice extracellulaire. Certaines de ces lésions inflammatoires, trop importantes ou trop brutales, vont conduire à des lésions qui vont se compliquer par une rupture ou une érosion de plaque et une thrombose artérielle. Les présentations cliniques de la maladie athéroscléreuse sont évidemment multiples et fonction de l'artère touchée. L'atteinte coronaire et l'infarctus du myocarde dominent par leur fréquence et leur sévérité suivis de près par les atteintes carotidiennes et les accidents vasculaires cérébraux. Mais la maladie reste souvent silencieuse et le patient le plus souvent asymptomatique. Les études épidémiologiques montrent cependant que le développement de l'athérosclérose ne doit pas être considéré comme un héritage inéluctable de l'industrialisation et de l'urbanisation.
Atherosclerosis is the leading cause of death in industrialized countries. It manifests as an arterial disease, involving a chronic inflammation related to the interaction between modified lipoproteins, inflammatory cells (macrophages and T lymphocytes) and cellular components of the arterial wall. This chronic inflammation leads to a reactional scarring process of the arterial wall, which involves the smooth muscle cells and the production of an extracellular matrix, resulting in the formation of complex lesions. Unfortunately, some of these arterial lesions are subject to complications. The rupture or erosion of a plaque followed by an arterial thrombosis results in the rapid obstruction of the vessel and the onset of major clinical events such as myocardial infarction or cerebral vascular events. However many patients remain asymptomatic. Epidemiologic studies have clearly demonstrated that adverse lifestyle changes accompanying industrialization and urbanization leading to the increase of cardiovascular diseases should not be considered inevitable. |
