第3度房室ブロック罹患犬の基礎心疾患と房室伝導系病変

Abstract 犬の第3度房室ブロック(AVB)症例の基礎心疾患と房室伝導系病変との関連性を明らかにするため,当該ブロック罹患犬36例の心臓を病理学的に検索した。基礎心疾患として,13例にリンパ球性非化膿性心筋炎(LyMy群),12例に房室弁心内膜症(AtEn群),各2例に先天性異常(CoAn群),心臓腫瘍(CaTu群),心房中隔下部への重度脂肪浸潤(FaIn群)が認められた。しかし,残りの5例に明らかな病的変化は見いだされなかった。房室結節アプローチ部(AVNap),房室結節(AVN),ヒス束貫通部(HisP)および分岐部(HisB),左脚(LBB)および右脚(RBB)からなる房室伝導系の組織学的検索では,基礎心疾患を有していなかった5例を除く31例すべてに伝導系細胞の脱落・消失が種々の程度に観察された。重度傷害がみられた部位は以下の通りである:LyMy群-AVNapからLBB/RBBまでのほぼ全域;AtEn群-HisPおよびHisB;CoAn群-HisP;CaTu群-AVNap,AVN,HisPおよび/あるいはHisB;FaIn群-AVNap。このように,犬における第3度AVB発生の形態学的基盤をなしていたのは,房室伝導系内の1箇所あるいは複数箇所における伝導系細胞の脱落・減数であり,これには房室接合部領域に重度の器質的障害をもたらす各種心疾患が密接に関わっていた。
